Billing & Insurance

There will be a $20 pre-payment charge for all forms that need to be completed. Due to the high volume of forms we receive, please allow 7 business days to get the form back to you. If there is a need for the form sooner, there is an additional $10 "rush" surcharge. Please fill out any patient specific sections prior to submission. All forms need to include instructions as to where to send the forms once completed, along with a self-addressed stamped envelope if they are to be mailed.

The Cardiovascular Centers participates with most major insurance plans. As a courtesy to our patients, our staff will automatically file your insurance claims. Please provide us with all necessary insurance data, including the name and address of your company and your policy number. All patients are requested to pay any required co-payments at the time of their office visit or the appointment will need to be rescheduled. For patients with high deductible health plans, defined as any plan with a deductible over $1,000, office will collect $100 toward their deductible at time of service.

If your insurance company requires a referral, please make sure to obtain this from your primary care physician prior to your appointment.

If you are uninsured, or experiencing financial difficulties, payment arrangements can be made available for qualified patients.
